Summary
The long term results of a 15 years time period will be presented.
From 1975 during 1988 170 bifurcation dacron grafts were implanted in 168 patients due to arterial occlusive disease. The ratio male: female was 2.8:1. The mean follow-up measured 7.41 years. The distal anastomosis was carried out as profundaplasty in 28.5% of the limbs, in 20.6% of the patients bilaterally.
The hospital mortality amounted to 5.9%, early mortality 2.9% and late mortality 28.8%. Uni- or bilateral graft occlusions occured in 33.52%, mostly due to the poor peripheral run off. 1975–1981 the proximal anastomotic technique still played a more important role. Immediate occlusion took place in 5.3%, early occlusion in 6.5% and late occlusion in 21.7%. Due to the cause of occlusion and the general surgical risk the surgical measures were wide spread from peripheral reconstructions up to extraanatomic bypass grafts.
The primary cumulative patency rate measured 87.5% after one year, 78.9% after three years, 70.1% after five and 61.5% after ten years. The secondary patency rates were 95.7%, 92.1%, 89.8% and 84.6% respectively. The cumulative survival rate amounted to 90.0%, 87.7%, 79.4% and 69.2% respectively at the same intervals. Finally the preservation of limbs was 91.8%, 89.9%, 85.9% and 84.4% respectively.
The long term results related to graft patency, survival rate and amputation rate seems better than in formerly reported papers. We conclude that there is no need for too restrictive management in performing aortofemoral bifurcation grafting.
Zusammenfassung
Die Langzeitergebnisse des Einsatzes einer aortofemoralen Y-Prothese aus einem bis zu 15jährigen Beobachtungszeitraum werden dargestellt. Von 1975 bis 1988 wurden bei 168 Patienten wegen AVK 170 Dacron-Y-Grafts implantiert. Das Verhältnis männlich: weiblich war 2,8:1. Das mittlere Follow-up betrug 7,4 Jahre. Die periphere Anastomose wurde bei 28,5% der Graftschenkel als Profundaplastik angelegt, in 20,6% der Patienten beidseitig.
Die Hospitalletalität betrug 5,9%, die Frühletalität 2,9% und die Spätletalität 28,8%. Ein- oder beidseitige Bypassverschlüsse traten bei 33,5% auf, überwiegend als Ursache eines schlechten peripheren Abflusses. 1975–81 spielte hierbei die Technik der zentralen Anastomose noch eine größere Rolle. Ein Sofortverschluß trat bei 5,3%, ein Frühverschluß bei 6,5% und ein Spätverschluß bei 21,7% auf. In Abhängigkeit von Verschlußursache und perioperativem Risiko des Patienten kam ein weites Spektrum an Reinterventionen — von extraanatomischen Umleitungen bis hin zur peripheren Rekonstruktion — zur Anwendung.
Die primäre Funktionsrate betrug nach einem Jahr 87,5%, nach drei Jahren 78,9%, nach fünf Jahren 70,1% und nach 10 Jahren 61,5%. Die sekundären Funktionsraten in denselben Zeitabschnitten betrugen 95,7%, 92,1%, 89,8% und 84,6%. Die kumulative Überlebensrate war 90,0%, 87,7%, 79,4% bzw. 69,2%. Der Extremitätenerhalt lag bei 91,8%, 89,9%, 85,9% und 84,4%.
