Der Cross-over-Bypass — primäres oder alternatives Verfahren zur Rekonstruktion der Beckenarterien?

Summary

The Operations generally carried out in surgical management of iliac artery occlusive disease are bypass procedure using prosthetic grafts and desobliteration of the occluded vascular segment.

As alternative procedure the crossing bypass has been established. Since 1964 this method of treatment has been used in our hospital in cases of the following indications: elderly and high risk patients, previous operations at the same level, occlusion of artificial graft limbs, infection, removal of infected transplanted kidney.

During this long observation time the mean follow up ranged up to 38 months with an occlusion rate of 14%. Therefore the crossing bypass has proven as justified measure for primary surgical reconstruction.

Zusammenfassung

Die allgemein angewendeten Operationverfahren zur Rekonstruktion der Beckenarterien sind der orthotope Bypass mit Kunststofftransplantaten und die Desobliteration des verschlossenen Gefäßabschnittes. Als alternatives Verfahren kommt der gekreuzte Bypass zur Anwendung.

Diese Operation wird bei uns seit 1964 vorgenommen. Die Indikationen dazu sind: hohes Alter der Patienten bei erhöhtem Operationsrisiko, Voroperationen, Prothesenverschlüsse, Infektionen bei einseitigen Rekonstruktionen, Entfernung von infizierten Transplantatnieren.

Im Beobachtungszeitraum seit 1980 betrug bei einer mittleren Follow-up-Dauer von 38 Monaten die Verschlußrate 14%. Der gekreuzte Bypass hat durchaus seine Berechtigung als Verfahren zur primären Gefäßrekonstruktion.
