Summary
We report on 252 femorofemoral reconstructions performed in cross-over technique which were carried out from 1985 through 1990. In 75% of all patients, an elective operation was carried out, and 25% underwent an emergency operation. The main indication in the group of electively operated patients was disabling claudication (66.1%), and in the group of emergency operations it was acute ischemia (81%).
The perioperative mortality was 1.6%. An early bypass occlusion was observed in 3.6%, and an amputation was necessary in 2.8% of all cases.
Eighty-three percent of all patients could be followed-up in 1991. The cumulative primary patency rate amounted to 67% after 4 years, and the secondary patency was 82%. Concerning septic complications, we observed five bypass infections (2%), two in the perioperative period, and three as a late infection.
Because of our results, we consider a femorofemoral cross-over bypass as being indicated in patients with unilateral occlusion of the iliac artery, especially in stages II and III according to Fontaine, as well as in the case of unsuccessful emergency thrombectomy.
Zusammenfassung
Wir berichten über 252 femorofemorale Rekonstruktionen in Cross-over-Technik, die in den Jahren 1985 bis 1990 durchgeführt wurden. Bei 75% der Patienten wurde eine elektive Operation und bei 25% ein Notfalleingriff durchgeführt. Die Hauptindikation in der Gruppe der elektiv operierten Patienten war mit 66,1% eine Claudi-catio intermittens, in der Gruppe der notfallmäßig Operierten eine akute Ischämie mit 81%.
Die perioperative Mortalität betrug 1,6%. Ein Bypassfrühverschluß wurde bei 3,6% beobachtet, eine Amputation war bei 2,8% der Patienten notwendig.
Bei der Nachuntersuchung im Jahr 1991 konnten 83% der Patienten erfaßt werden. Die kumulative primäre Durchgängigkeitsrate betrug nach 4 Jahren 67% und die sekundäre Durchgängigkeitsrate 82%. Was die septischen Komplikationen betrifft, so haben wir eine Bypassinfektion bei 5 Patienten (2%) beobachtet (2 Früh-und 3 Spätinfektionen).
Aufgrund unserer Ergebnisse ist bei einseitigem Beckenarterienverschluß die Indikation zum femorofemoralen Cross-over-Bypass v.a. im Stadium II und III der pAVK sowie bei erfolgloser notfallmäßiger Thrombektomie gegeben.
