Zusammenfassung
Die operative Rekonstruktion der infrarenalen Aorta durch Implantation einer Gefäßprothese (P) stellt die Standardtherapie zur Behandlung des Bauchaortenaneurysmas (BAA) dar. Bei diesem Eingriff kommt es regelmäßig durch Eventeration und Aortenclamping zur Ausschüttung von Mediatoren und aktivierten Leukozyten aus dem Darm und aus der reperfundierten unteren Körperhälfte [1–5]. Als weniger belastendes Behandlungsprinzip wurde daher in den letzten Jahren die transfemorale Implantation beschichteter Gefäßstents (S) entwickelt. Hierbei entfällt die Eventeration, die Ischämie betrifft nur eine Extremität und ist (wegen des peripheren Klemmensitzes) weniger vollständig, jedoch (technisch bedingt) meist deutlich länger. Die Auswirkungen dieses alternativen Verfahrens auf die Freisetzung von Mediatoren und Adhäsionsmolekülen wurden vergleichend untersucht.
