Zusammenfassung
Ziel der bilateralen Implantation asymmetrischer Multifokallinsen (MIOL) ist eine Verbesserung der Kontrastempfindlichkeit in der Ferne und Nähe gegenüber herkömmlichen MIOL.
Material und Methodik: Bei 12 Patienten wurde nach bilateraler Kataraktchirurgie eine neue diffraktive Silikon-MIOL mit unterschiedlicher Gewichtung der Lichtenergie für den Fern- und Nahfokus implantiert. Ein Auge erhielt eine ferndominante MIOL mit einer Lichtverteilung von 70 % für die Ferne und 30 % für die Nähe und das Gegenauge eine nahdominante MIOL mit umgekehrter Lichtverteilung.
Postoperativ wurden mon- und binokular der unkorrigierte und korrigierte Fernvisus und Nahvisus nach Nieden und der Kontrastvisus an den Kontrasttafeln nach Regan geprüft. Der binokulare Kontrastvisus wurde mit einer Gruppe monofokal pseudophaker Patienten verglichen.
Ergebnisse: Es zeigt sich eine Visusverbesserung im dominanten Fokus der jeweiligen MIOL gegenüber dem nichtdominanten Fokus. Bei binokularer Prüfung erzielten 7 Patienten (64%) einen Visus von 1,0 oder besser und 8 Patienten (73 %) einen Nahvisus von Nieden 1. Der binokulare Kontrastvisus war nach MIOL-Implantation bei der geringsten Kontraststufe (11%) signifikant reduziert gegenüber dem Vergleichskollektiv mit Monofokallinse.
Schlußfolgerung: Die bilaterale Implantation asymmetrischer MIOL stellt ein wirksames Verfahren zur Herstellung einer künstlichen Akkommodationsfähigkeit und Verminderung der Kontrastreduktion von MIOL gegenüber monofokalen IOL dar.
Summary
Thè aim of bilateral multifocal intraocular lens implantation (MIOL) with asymmetrical light distribution is to improve contrast sensitivity at distance and at near compared to conventional MIOLs.
Patients and methods: A new type of diffractive silicone MIOL was bilaterally implanted in 12 patients after cataract surgery. One eye received a distant-dominant MIOL with a light distribution of 70 % for the far and 30 % for the near focus and the fellow eye a near-dominant MIOL with an opposite light distribution. Uncorrected and corrected distance and near visual acuity as well as contrast acuity using the Regan contrast charts were tested postoperatively. Binocular contrast acuity was compared with a control group of monofocal Pseudophakic patients.
Results: The results show better visual acuity in the dominant focus of either MIOL than in the non-dominant focus. Seven patients (64%) achieved a binocular distance visual acuity of 1.0 and eight patients (73%) a near visual acuity of Nieden 1. The binocular contrast acuity was significantly lower at the lowest contrast level (11%) in the MIOL group than in the monofocal group.
Conclusion: The concept of bilateral MIOL implantation effectively provides both distance and near visual acuity without bifocal spectacle correction and reduces the loss of contrast sensitivity in MIOL compared to monofocal IOL.
