Zusammenfassung
Im Rahmen der klinischen Tätigkeit im Operationssaal stellt die Anästhesie für geburtshilfliche Eingriffe eine Besonderheit dar. Nicht nur die Mutter, sondern auch der Fet bzw. das Neugeborene werden den Wirkungen der Narkose und ihrer Adjuvanzien ausgesetzt. Bei narkotisch voll wirksamer Dosierung für die Mutter üben praktisch alle heute gebräuchlichen Anästhetika einen mehr oder weniger ausgeprägten narkotischen Effekt auch auf das Neugeborene aus. Eine gewisse Einsparung von Anästhetika und damit eine Reduktion von Sekundäreffekten beim Feten bzw. Neugeborenen kann durch die Anwendung der Muskelrelaxanzien erreicht werden. Die Indikation zum Einsatz dieser Substanzen unterscheidet sich in der Geburtshilfe prinzipiell nicht von der Anwendung in anderen operativen Bereichen. So dienen sie zur Durchführung der Intubation, zur Aufrechterhaltung der Narkose unter Einsparung von Anästhetika, zur Erleichterung einer adäquaten Beatmung und zur Relaxierung der Bauchmuskulatur. Eine gute Entspannung der Beckenbodenmuskulatur bei operativen vaginalen Entbindungen beschleunigt die Entwicklung des Kindes und verhindert schwere Verletzungen der unteren Geburtswege mit all ihren nachfolgenden Komplikationen.
