Coxsackievirus Genetics and the Cardiovirulent Viral Phenotype

The Role of Immune Mechanisms in Cardiovascular Disease

Abstract

The coxsackie B viruses are a group of typical human enteroviruses (family Picornaviridae) that are comprised of six serotypes (CVB1-6). The CVBs are common etiologic agents of childhood disease including inflammatory diseases of the heart muscle and of the central nervous system and are the cause of a significant amount of adult inflammatory heart muscle disease (Martino et al. 1995). Dilated cardiomyopathy (DCM) has been linked to enterovirus infection of the heart through a variety of evidence, including serological data as well as molecular detection of viral RNA in diseased heart tissue samples (Keeling and Tracy 1995). A survey of results from a variety of studies suggest that, on average, about 20%–25% of DCM hearts show enteroviral involvement on the basis of polymerase chain reaction (PCR) or in situ hybridization data (Martino et al. 1995). Thus, many cases of both acute myocarditis as well as DCM may be considered as infectious viral diseases.
