Zusammenfassung
Der Begriff für die Substanzklasse der „Nootropika“ wurde vor knapp 30 Jahren auf der Basis der damals neuartigen pharmakologischen und klinischen Eigenschaften der Substanz Piracetam geprägt (Giurgea, 1976; Giurgea et al. , 1981). Obwohl Piracetam ein relativ einfaches zyklisches Derivat des inhibitorischen Neurotransmitters GABA darstellt, ist der Wirkungsmechanismus von Piracetam in keiner Beziehung zur physiologischen Funktion von GABA zu sehen. Piracetam wurde ursprünglich in einem Modell für „zentralen Nystagmus“ getestet, in dem zu dieser Zeit hauptsächlich anticholinerge und antihistaminerge Substanzen aktiv untersucht wurden (Giurgia, 1976 u. 1982). Der therapeutische Einsatz von Piracetam bei Vertigo unterschiedlicher Genese (Oostervelt, 1999) geht wahrscheinlich noch auf diese Eigenschaften zurück. Die Suche nach dem pharmakologischen Wirkprinzip von Piracetam gestaltete sich aber sehr schwierig, da diese Substanz nicht nur weder anticholinerg oder antihistaminisch wirksam war, sondern auch in praktisch allen Tiermodellen keine ausreichende Wirksamkeit zeigte, die als beweisend für zentral sedierende, zentral aktivierende oder das vegetative Nervensystem beeinflussende Eigenschaften galten (Giurgia, 1976; Giurgia et al. , 1981). Als neuartige pharmakologische Eigenschaften konnte gezeigt werden, daß sich unter Piracetam die interhemisphärische Kommunikation verbesserte, daß Piracetam die Widerstandsfähigkeit des Gehirnes gegenüber verschiedenen Noxen (z. B. Hypoxie) verstärkte und daß es vor allen Dingen Lehr- und Gedächtnisleistungen verbessern konnte, besonders in Situationen, in denen diese Eigenschaften experimentell beeinträchtigt waren (Giurgia, 1976). Da keine andere Vergleichssubstanz zu dieser Zeit ähnliche pharmakologische Eigenschaften zeigte, wurde Piracetam die erste Substanz der neuen Klasse der „Nootropika“ (Vernon u. Sorkin, 1991; Guliaef u. Senning, 1994; Nobel u. Benfield, 1998).
