Zusammenfassung

Informationen über den Verlauf der Alzheimer Demenz (AD) sind sowohl für die Patienten und ihre Familien von Bedeutung, als auch für den behandelnden Arzt. Die direkt und indirekt Betroffenen müssen wissen, welche Schwierigkeiten auf sie zukommen werden und wann dies etwa geschehen wird, damit sie sich auf die Krankheit einstellen und die Versorgung des Kranken planen können. Der Arzt muß die zu erwartende Geschwindigkeit des Verlustes an kognitiver Leistungsfähigkeit und Alltagskompetenz kennen, um seine Diagnose zu überprüfen und um den Erfolg seiner Therapie zu beurteilen. Die in diesem Beitrag vorgestellten Befunde beziehen sich, falls nicht anders angegeben, auf zwei Studien, die am Alzheimer-Zentrum der Technischen Universität München durchgeführt wurden. Die erste davon war eine Longitudinalstudie an 88 Patienten, die in zwölfmonatigen Abständen über bis zu 3 Jahre nachuntersucht wurden (Kurz et a., 1992). Die zweite Studie war eine Langzeit-Katamnese an 178 zwischen 1985 und 1990 erstuntersuchten Patienten mit AD zur Ermittlung der Überlebenswahrscheinlichkeit und der Häufigkeit von Heimeinweisungen.
