Zusammenfassung

D ie perkutane transluminale Angioplastie supraaortaler hirnversorgender Arterien wird bereits seit mehr als 20 Jahren praktiziert, wobei die kontroversen Diskussionen um die klinische Wertigkeit der interventionellen Behandlung noch nicht abgeschlossen sind. Während beispielsweise die Überlegenheit der Operation von hochgradigen, neurologische Symptome verursachenden Karotisstenosen gegenüber einer medikamentösen Behandlung durch große randomisierte Studien [11, 28] belegt ist, fehlen derartige Daten für die endovaskuläre Therapie supraaortaler Stenosen. Dennoch werden Angioplastien insbesondere an der Karotis mit zunehmender Häufigkeit als Alternative zur etablierten gefäßchirurgischen Behandlung durchgeführt. Die primäre Stentimplantation ist dabei heute meist die Methode der Wahl, so dass es nicht sinnvoll ist, die immer weniger angewandte alleinige Ballondilatation getrennt abzuhandeln.
