Zusammenfassung

Die von J. Simpson entwickelte direktionale koronare Atherektomie [47] wurde nach einer vorklinischen Erprobungsphase im Jahre 1989/90 für die interventionelle Therapie arteriosklerotischer Koronarstenosen zugelassen [16,41]. Mit der direktionalen koronaren Atherektomie stand erstmals ein nichtchirurgisches, perkutan arbeitendes Verfahren zur Verfügung, das ein partielles oder vollständiges Entfernen von Plaquematerial aus Koronararterien ermöglichte. Mit dieser neuen interventionellen Technik verbanden sich hohe Erwartungen, vor allem hinsichtlich einer Reduktion der persistierend hohen Rezidivstenoserate nach PTCA, durch die Möglichkeit, ein größeres postinterventionelles Lumen zu erzielen. Theoretisch sollte die verbliebene Gefäßoberfläche der „target lesion“ nach Atherektomie glattwandiger als nach Ballonangioplastie sein. Das elastische „recoil” - ein wesentlicher Pathomechanismus der Entwicklung einer Rezidivstenose nach PTCA - sollte durch die geringen Ballondrücke des Atherektomiekatheters vermieden werden.
