Behandlung von entzündlichen Adnexerkrankungen — Laparoskopie versus Laparotomie

Zusammenfassung

Die vorhandene Literatur über die operative Therapie von entzündlichen Adnexerkrankungen beschäftigt sich vornehmlich mit der Therapie des Tuboovarialabszesses bzw. des pelvinen Abszesses und stammt zumeist aus den Jahren 1975-1994. Das Management des Tuboovarialabszesses wird kontrovers diskutiert und reicht von der konservativen Behandlung mit Antibiotika bis hin zur sofortigen Laparotomie [23]. Die Entwick-lung von der klassischen chirurgischen Sanierung per laparotomiam mit Hysterektomie und Adnektomie beiderseits bis hin zum gegenwärtig be-vorzugten fertilitätserhaltenden Vorgehen mittels Laparoskopie ist eng mit der Entwicklung von neuen, hochwirksamen Antibiotika verbunden.Diverse Autoren empfehlen einen konservativen Therapieversuch mit Breitbandantibiotika und nur bei Therapieversagen oder ABszessen über 8 cm eine chirurgische Intervention [40 49].Die Erfolgsrate der rein antibiotischen Therapie schwankt innerhalb der durchgeführten Studien zwischen 16 und 81%.Seit über 15 Jahren stellt die operative Laparoskopie den bevorzugten Zugangweg dar [23 29 36 39 44]. Empfohlen wird eine Intervntion innerhalb der ersten 24 Stunden nach stationärer Aufnahme bzw. nach Beginn der Antibiose. Prospektive randomisierte Studien der einzelnen Therapieansätze liegen bis dato nicht vor. Ein Vergleich der unterschiedlichen operativen Zugangswege, bezüglich der Zielgrößen, Erfolgsrate, Komplikationen oder postoperative Schwangerschaftstrate, wird weiterhin durch die unterschiedliche Radikalität der Eingriffe und die unterschiedlichen antibiotischen Behandlungsschemata erschwert.
