Zusammenfassung
Ein Blick in die Weltliteratur macht deutlich wie spät (selten in den 60er, häufiger in den 70er Jahren) das Schultergelenk bei Polyarthritis das Interesse auf sich zog [4, 6, 7, 10, 15, 21, 23-29, 32, 33, 38]. Dies hat verschiedene Gründe, von denen nur die wichtigsten erwähnt seien: Im Vergleich zu den meisten anderen Gelenken (vor allem Knie, Füße und Hände) wird das Schultergelenk mehrheitlich erst einige Jahre nach Beginn der Krankheit befallen, wenn sich die Aufmerksamkeit von Arzt und Patient bereits brennenderen Problemen zugewandt hat. An der oberen Extremität besteht im Gegensatz zu den Gelenken der unteren Extremität die Möglichkeit der funktionellen Kompensation durch eine weniger schwer betroffene Gegenseite; auch können die meisten vitalen Funktionen (vor allem Essen) selbst bei fortgeschrittener Zerstörung und Steifheit solange ausgeübt werden, wie die Ellbogenflexion erhalten bleibt. Dies ist nach einer Studie an 300 Polyarthritispatienten, bei denen wir die Ellbogenbeweglichkeit gemessen haben, bei 87 % auch dann noch der Fall, wenn das Ellbogengelenk radiologisch fortgeschrittene Veränderungen aufweist.
