Zusammenfassung
Eine optimal aufeinander abgestimmte Position von Pfanne und Schaft sind für die Luxationssicherheit einerseits und die Langzeitstabilität andererseits wesentliche Voraussetzungen [1]. In den letzten fünf Jahrzehnten wurden zwar gravierende Fortschritte beim Hüftgelenksersatz im Hinblick auf Materialentwicklung, Verankerungstechnik, Abriebminimierung und überlebenszeit erzielt [1, 20], dennoch ist bis heute eine präzise, individuell optimierte Pfannenpositionierung nicht sicher zu erreichen und immer von der Erfahrung und dem Augenmaß des Operateurs abhängig. Ungünstige Pfannenstellungen können zu Bewegungseinschränkung, Impingement [8], erhöhter Abriebrate [3, 11] und zum Risiko der postoperativen Luxation führen, deren Häufigkeit je nach Autor mit 1–9% angegeben wird [1, 6]. Die “safe zone” [16] mit 40 ± 10° für die Inklination und 15 ± 10° für die Anteversion wird in konventioneller Technik selbst von erfahrenen Operateuren in bis zu 42% nicht erreicht [9]. Dabei kann die Winkelstellung der Pfanne von 14–65° für die Inklination und von 27° Retro- bis 47° für die Anteversion betragen [15].
