Klassifikation von Mißbrauch und Abhängigkeit: Diagnostik aus psychiatrischer Sicht

Zusammenfassung

Die Begriffe Alkoholismus und Sucht sind in modernen psychiatrischen Klassifikationssystemen durch genauer definierte und operationalisierte diagnostische Kategorien wie Mißbrauch (bzw. schädlicher Gebrauch) sowie Abhängigkeit ersetzt worden (1, 34). Sie basieren auf dem Konzept von Edwards und Gross (9), nach denen das Abhängigkeitssyndrom ein Cluster von physiologischen, somatischen und psychischen Symptomen darstellt, von denen einige, aber nicht alle für die Diagnosestellung erforderlich sind. Im klinischen Sprachgebrauch haben aber auch die Begriffe Alkoholismus (oft gebraucht als Synonym für Mißbrauch und Abhängigkeit), Alkoholiker (statt Alkoholabhängige, Alkoholkranke), sowie Sucht ihren Stellenwert behalten. So tragen z.B. wichtige Fachzeitschriften noch entsprechende Namen. Bevor eine Definition der einzelnen Begriffe und speziell der Mißbrauchs- und Abhängigkeitskriterien in ICD 10 und DSM IV gegeben werden, sollen einige grundsätzliche Ausführungen zu Aufgaben und Ziel der psychiatrischen Diagnostik Alkoholkranker gemacht werden.
