Ambulante Entgiftung und Entwöhnung von Alkohol — Probleme bei Diagnostik und Risikoabschätzung sowie aktuelle Ergebnisse der Therapieforschung

Zusammenfassung

Während stationäre Entwöhnungstherapien vergleichsweise gut untersucht sind, liegen bislang nur wenige Untersuchungen zur Effizienz ambulanter Entgiftungs- und Entwöhnungstherapien bei Alkoholabhängigen vor. Es wurden sowohl neue Methoden der ambulanten Entgiftung (1, 26) als auch ambulante Therapien (18) propagiert, teilweise in Kombination mit Disulfiram oder psychotropen Medikamenten (15). Die therapeutischen Erfahrungen sind bislang sehr begrenzt. In den letzten Jahren sind allerdings aussichtsreiche Bemühungen erkennbar geworden, vermehrt rein ambulante oder teilstationäre Behandlungen bei Alkoholabhängigen zu etablieren (3, 5). Argumente für solche Therapien sind u.a. die vergleichsweise geringen Kosten und die Möglichkeit berufs- oder familienbegleitender Therapiemaßnahmen. Die psychiatrische Klinik der Universität München hat in Zusammenarbeit mit der Fachambulanz zur Behandlung von Suchterkrankungen Untersuchungen zur Effizienz ambulanter Therapien durchgeführt, die im folgenden näher dargestellt werden sollen.
