Die proximale Humerusfraktur

Zusammenfassung

Die primäre Diagnostik besteht nach einer ausführlichen Anamnese in der klinischen Untersuchung, welche in Abhängigkeit von der akuten Schmerzsituation des Patienten durchzuführen ist. Aufgrund der frakturbedingten Einblutung in das Gelenk und umgebende Weichteile schwillt die Schulter an, und die Schulterkontur verstreicht. Ein ausgeprägter Weichteilmantel lässt diese Veränderung meist nur erahnen. Nach 3–5 Tagen entwickelt sich daraus die typische livide Hämatomverfärbung, welche sich mit der Schwerkraft nach distal ausweitet. Zum Ausschluss offener Frakturen gehören die Beurteilung des Haut-Weichteil-Mantels und das Erkennen von Deformitäten, welche auf Begleitverletzungen hinweisen können.
