Zusammenfassung
Psychosoziale Faktoren wie z. B. chronischer Stress im Alltag, eine depressive Stimmungslage oder eine ungünstige Schmerzverarbeitung können sowohl an der Entstehung als auch, und dies im Besonderen, an der Aufrechterhaltung von Schmerzen beteiligt sein. Sie tragen dazu bei, dass z. B. akute Kreuz- oder Nackenschmerzen trotz adäquater medizinischer Maßnahmen nicht angemessen ausheilen, d. h., Betroffene versäumen ihre Genesungsphase, ohne dass sie dies bewusst erleben. Hinzu kommen Folgen anhaltender Schmerzen wie z. B. Schlafstörungen, vermehrte Konzentrationsprobleme, erhöhte muskuläre Anspannungen, Ängste und weitere depressive Verstimmungen. Diese Faktoren sollten niemals in Form einer Ausschlussdiagnostik – d. h. wenn keine organischen Schädigungen feststellbar sind – »erfasst« werden, sondern immer anhand zuverlässiger diagnostischer Maßnahmen. Erste Orientierungen sollten in allen Stadien einer Schmerzerkrankung realisiert werden, beim akuten Schmerz in der ärztlichen Praxis, im subakuten Stadium zudem im physiotherapeutischen Setting, im chronischen Stadium in der multiprofessionellen Zusammenarbeit von Arzt, Physiotherapeut und Psychotherapeut. Dieser Beitrag gibt einen Einblick in die verschiedenen diagnostischen Schritte psychosozialer Faktoren, die sich an den biopsychosozialen Krankheitsmodellen sowie an der psychosozialen multiaxialen Schmerzklassifikation (MASK-P) orientieren.
