Zusammenfassung

Die Nebenschilddrüsen stellen das Serumkalzium im Sinne einer "biologischen Konstante" ein. Mittels der raschen Anpassung der Parathormonsekretion justieren sie das Serumkalzium auf einen Sollwert. Jede Abweichung des Serumkalziums nach oben oder nach unten zeigt eine Fehlfunktion dieses Regelkreises an und bedarf einer Abklärung. Die Beurteilung dieses Regelkreises ist nur durch eine gleichzeitige Bestimmung des Parathormons und des Serumkalziums möglich. Beim primären und tertiären Hyperparathyreoidismus kommt es durch eine Übersekretion von Parathormon (PTH) zu einer Hyperkalzämie, beim sekundären Hyperparathyreoidismus versuchen die Nebenschilddrüsen, durch eine Erhöhung der PTH-Sekretion einen Kalzium oder Vitamin-D-Mangel auszugleichen – hier ist das Serumkalzium erniedrigt.
