Zusammenfassung
Die Diagnostik und Behandlung des akuten Herzinfarkts hat sich in den letzten 30 Jahrengrundlegend geändert. Beim ST-Hebungsinfarkt war bis zum Beginn der medikamentösen Reperfusionsbehandlung durch Thrombolyse nur eine konservative Therapie durch Analgesie, kardioprotektive Medikamente, Überwachung und eine mehrere Wochen dauernde Bettruhe möglich. Erst mit der Erkenntnis, dass ein Herzinfarkt durch eine Gerinnselbildung über einer instabilen Gefäßläsion entsteht, und aufgrund der therapeutischen Anwendung von thrombolytisch wirksamen Medikamenten wurde eine hochsignifikante Reduktion der Sterblichkeit um bis zu 50 Prozent erreicht. Heute schon ist die thrombolytische Therapie nur noch eine Ersatzstrategie, während die katheterbasierte Rekanalisationsbehandlung als Standard gelten muss, mit einer weiteren Reduktion der Sterblichkeit um bis zu 30 Prozent gegenüber der thrombolytischen Therapie. Die Fortschritte der Therapie schlagen sich auch in einer erheblichen Reduktion der Verweildauer der Infarktpatienten von sieben Tagen im Mittel nieder.
Beim Nicht-ST-Hebungsinfarkt hat die Einführung des kardialenTroponins als diagnostischer Standard die Risikostratifizierung von Patienten mit Thoraxschmerz und die therapeutischen Strategien bei diesem Patientenkollektiv maßgeblich beeinflusst. Auch für diese Patienten mit kleineren Herzinfarkten konnte die Überlegenheit einer frühzeitigen Therapie durch Ballondilatation und Stentimplantation gezeigt werden.
Es ist also die frühzeitige und kompetente interventionelle Behandlung von Patienten mit Herzinfarkt die Therapie der Wahl. Für die optimale klinische Versorgung erforderte dies die Einrichtung hochspezialisierer Einheiten, wie sie modellhaft und einmalig in Deutschland mit der integrierten Einheit von Chest Pain Unit, Herzkatheterlabor und kardiologischer Intensivstation in der Medizinischen Klinik in Heidelberg umgesetzt wurde. So können in der Diagnostik und Behandlung des akuten Herzinfarkts Erfolge nachgewiesen werden, die bei anderen lebensbedrohlichen Erkrankungen noch ausstehen.
