Abstract

The expert role is the central role of the social and health professional. Health and social care professionals possess a defined body of knowledge, disciplinary and procedural skills and attitudes, which are directed towards providing optimal person-centred support for health and wellbeing from their own particular scope of practice. They form a collaborative relationship with the older person and their family. The interrelatedness of physical, mental, social and spiritual aspects is characteristic for the older persons, and this makes care and support of older people demanding and complex. A systematic approach starts with an extended assessment and observations. These give information of the needs of older persons and their goals. This is the basic of a planning and of interventions to achieve these goals. A frequently evaluation is needed to control the effects’ and the eventually side effects of the interventions and the state of the older people.
