Abstract
Chronic venous insufficiency is a major health problem that affects many people with devastating consequences. Venous valvular incompetence leading to venous reflux and venous hypertension is the pathophysiology of developing venous disease. Increasingly, incompetent perforator veins have been recognized as a contributor to recalcitrant or recurrent venous ulcers. Historically, surgical interruption of perforators has been performed and shown to improve healing of venous ulcers. With advances in technology and comfort in endovenous procedures, percutaneous thermal ablation and ultrasound-guided foam sclerotherapy have gained popularity in the treatment of perforating veins with good success.
