Review and document the following:

  • Chief complaint

  • Pertinent medical and surgical history

  • History of allergies including type and severity of allergic reaction

  • Review of organ systems

  • Pertinent laboratory results and imaging studies

  • Current medications (prescribed and illicit)

  • Directed physical examination

  • Determine cardiopulmonary stability and request subspecialty consultation as necessary

  • Review sedation or anesthesia requirements and request anesthesia consultation as necessary

Additional for Angiographic Procedures [1, 2]

Evaluate and document the following:

  • Proposed puncture site and contraindications (e.g., groin infection, common femoral artery aneurysm/pseudoaneurysm, inguinal hernia, fresh incision, and recent injury)

  • Extremity pulses, using a Doppler ultrasound probe when necessary

  • Status of extremities (e.g., color, perfusion, presence of swelling, ulceration)

  • Risk factors for contrast allergy (e.g., previous allergic reaction to iodinated contrast, reaction to other allergens), and type/severity of allergy

  • Risk Factor for contrast-induced nephropathy (e.g., preexisting renal dysfunction, diabetes, dehydration, hypotension, congestive heart failure, large contrast dose, advanced age, anemia, cardiovascular diseases, multiple myeloma)
