The Lacrimal System

Abstract

Powered endoscopic dacryocystorhinostomy allows the lacrimal sac to be fully exposed so that it stands proud of the lateral nasal wall after dissection while avoiding an external incision and scar. The results of this technique have improved over the past few decades and rival or even surpass the open approach. By fully preserving all the lacrimal mucosa during opening of the sac, the sac can be marsupialized into the lateral nasal wall becoming part of the lateral nasal wall. This chapter outlines the details of completing a powered endoscopic dacryocystorhinostomy. The brief history and evolution of the procedure is described in addition to the nuances of the technique. Results of this procedure have proved to be reliable in primary, revision, and in pediatric DCRs.
