Abstract
Intracranial atherosclerotic disease (ICAD) is responsible for a significant proportion of ischemic strokes worldwide. Conservative medical management is the appropriate first step in the treatment of ICAD. The clinical presentation of ICAD is heterogeneous, with ischemia resulting from more than one mechanism. Delineating the mechanism of ischemia needs careful clinical analysis and usually necessitates multimodal imaging. An endovascular treatment approach based on the mechanism of stroke may be beneficial for select patients. Patient selection will be a critical factor in the design of future ICAD clinical trials.
