Abstract

EUS-guided cholangiodrainage (EUS-BD) is the standard procedure for internal bile drainage in the palliative setting even before PTCD in the case of failed ERCP in hospitals with high expertise. In addition to the internal antegrade drainage technique, antegrade extrahepatic cholangioenterostomy and transhepatic retrograde hepaticoenterostomy are available, as well as EUS-guided gallbladder drainage (EUS-GBD) in high-risk patients with acute cholecystitis. In patients with remitting pancreatitis and retention of the pancreatic duct following pancreatic resection, EUS-guided transgastric pancreatic drainage (EUS-PD) can achieve sufficient drainage and avoid the need for repeat surgery. EUS-guided tumor therapy with instillation of alcohol or MWTA of neuroendocrine tumors is a safe alternative to surgery for symptomatic high-risk patients. EUS-guided therapy with application of coils or instillation of fibrin directly into the vessels may be an alternative to angiographic intervention for recurrent bleeding from tumors or acute bleeding from a pseudoaneurysm.
