5-FU Induced Atrial Fibrillation in the Context of Ischemic Heart Disease

Abstract

Patient was a 71-year-old male with metastatic HER2-negative gastric adenocarcinoma, and placement of gastric stent, previously on FOLFOX chemotherapy with oxaliplatin cessation due to painful neuropathy, currently on modified de Gramont 5-FU [de Gramont et al. (Eur J Clin Oncol 24:1499, 1988)]. He presented three times to the Emergency department with rapid heart rate. He was found to be in atrial fibrillation with heart rate of 120 beats per minute. He was started on direct oral anticoagulation and verapamil. On his last presentation to emergency department, he had troponin rise associated with chest tightness. He underwent successful electrical cardioversion.

While admitted in hospital, given raise in troponin, he underwent successful PCI of the mid LAD and proximal RCA. After PCI, there was no recurrence of chest pain and clinical AF.

We think the occurrence of AF in this patient was multifactorial due to a combination of his underlying comorbidities, coronary artery disease triggered by 5FU.
