The Non-Disclosing Patient

Abstract

When conceptualized within the frame of the “transactional interview,” nondisclosure in its many forms represents a threat to the integrity of the assessment process, yet one that affords a unique opportunity to explore the motivational forces driving a patient’s behavior. This opportunity can best be realized when the interviewer (a) is guided by a developing clinical formulation, (b) utilizes his or her own experiential responses to the interview to access nondisclosed domains of the patient–clinician relationship, (c) focuses on the motivation driving nondisclosure rather than content of the nondisclosed material, and (d) focuses on emotions over facts.
