Abstract

The primary objectives in the treatment of displaced intra-articular calcaneal fractures are to restore not only the posterior facet articular surface but also calcaneal height, length, and overall morphology, so as to maximize the function and longevity of the subtalar joint. The calcaneus plays an integral role in hindfoot motion and function in accommodating uneven ground and indirectly influences ankle motion. Weight-bearing radiographs and CT scan provide invaluable information as to the original fracture pattern, extent of displacement, and subtalar joint salvageability. Certain malunited fractures can be re-created along the original fracture lines and corrective techniques employed to restore alignment and preserve subtalar joint motion. In many instances, the subtalar joint is not salvageable and must therefore be sacrificed as part of the reconstructive salvage.
