Preoperative Management of Oral Anticoagulants

Abstract

The oral anticoagulants vitamin K antagonists, factor Xa-inhibitors (rivaroxaban, apixaban, and edoxaban), and thrombin inhibitors (dabigatran etexilate) are increasingly used among cardiac surgery patients. While the mechanistic action of these inhibitors differs, they are all associated with a risk for perioperative bleeding, and preoperative discontinuation of these agents requires individual titration. Alternatively, the action of oral anticoagulants can be reversed by prothrombin complex concentrate, idarucizumab, and andexanet alfa in case of urgent procedures.
