Résumé

La sénologie interventionnelle est proposée comme alternative à la chirurgie diagnostique. Les biopsies permettent d’éviter des chirurgies inutiles et de rassurer les patientes en cas de bénignité. En cas de malignité, elles permettent de confirmer rapidement le diagnostic, d’obtenir les principaux éléments pronostiques et l’élaboration du projet thérapeutique. Si les biopsies ont un rôle essentiel pour « le diagnostic », certaines techniques, telles que les macrobiopsies, par leur capacité d’exérèse volumique peuvent également avoir un rôle « thérapeutique », ce dernier restant encore limité à l’exérèse de lésions bénignes telles que les fibroadénomes ou les papillomes. Depuis plusieurs années, chirurgiens et radiologues proposent des techniques interventionnelles mini-invasives, fondées sur l’hyperthermie ou la cryothérapie, dans un rôle exclusivement thérapeutique sans exérèse de la lésion mais fondées sur sa destruction in situ.
