Abstrait

L’antibiothérapie des infections urinaires (IU) s’envisage très différemment selon qu’il s’agit d’un schéma préventif ou curatif et selon la situation clinique. Le choix est bien codifié pour l’antibioprophylaxie et pour l’antibiothérapie probabiliste des IU communautaires, les espèces en cause étant très stables dans le temps et dans l’espace, et la prévalence de la résistance étant largement documentée (prévalence désormais élevée non seulement pour les aminopénicillines, mais aussi pour l’association amoxicilline-acide clavulanique et pour le cotrimoxazole); a contrario, le traitement des IU nosocomiales est rendu complexe par la diversité des germes en cause, tant en terme d’espèce que de résistance.
