Abstract

It is not so long ago that the right ventricle was considered very much secondary to the left ventricle, in terms of both its influence on the normal circulation, and its impact on outcomes in acquired heart disease. The past two decades however, have seen a remarkable change in our understanding of its role. Fundamental to the normal circulation, the influence of heart–lung interactions, electro–mechanical interactions, and right–left heart interactions, have all been implicated as therapeutic targets in different forms of heart disease. Consequently, the important role of right ventricular dysfunction in determining the outcome of a variety of “left heart” diseases is now established. For example, it is now known that survival in dilated left ventricular cardiomyopathy is markedly influence by the co-existence of right ventricular dysfunction.1
