Management of Myocardial Reperfusion Injury

Abstract

In the past, the clinical significance of myocardial reperfusion injury in man has been questioned. However, the recent discovery of therapeutic interventions, which can reduce myocardial infarct size in STEMI patients undergoing PPCI when administered at the time of myocardial reperfusion, has confirmed the existence of myocardial reperfusion injury in man and provided a new frontier for the treatment of myocardial infarction. Concurrent with these developments, recent advances in cardiac MRI have provided the ability to directly visualize the effects of myocardial reperfusion injury and assess the efficacy of novel cardioprotective strategies in PPCI patients. A number of novel cardioprotective strategies have shown promise in proof-of-concept clinical studies. Large multi-centre randomized controlled clinical trials are now required to investigate whether targeting myocardial reperfusion injury can improve clinical outcomes in patients with ischemic heart disease.
