Abstract

High blood pressure (BP) is a major modifiable risk factor for stroke. Though the role of BP lowering in the long-term care of subjects with an ischemic stroke is well established, BP management in the acute setting is controversial, largely due to concerns about the possibility of worsening ischemia by BP lowering. This chapter discusses the pros and cons of lowering BP in this setting, summarizes the studies on cerebral blood flow and clinical outcome after BP lowering in acute stroke and lists guidelines for BP management after acute stroke.
