Training in Endoscopic Retrograde Cholangiopancreatography

Abstract

Endoscopic retrograde cholangiopancreatography (ERCP) is a powerful diagnostic and therapeutic tool in the management of pancreaticobiliary diseases. The maturation of sophisticated imaging technologies has caused ERCP to become primarily a therapeutic modality. ERCP demands a level of intensity and dedication in training that is rarely feasible during a 3-year general gastroenterology fellowship, and thus the advanced fellowship has become an increasingly popular and endorsed method for ERCP training. This allows training in the full spectrum of techniques in ERCP from the basic to the most complex. Training in ERCP develops competence in its many cognitive and technical components. Of note, data informing training methods or volume are scarce and limited mostly to selective cannulation and sphincterotomy. Use of simulators in ERCP training is a relatively recent development with promising supporting data, and as these devices increase in sophistication, they may become integrated into training programs in a more widespread fashion. Increasing attention to quality indicators in ERCP practice informs the assessment of fellows as they proceed through training and particularly as they approach graduation. The current paradigm focuses on case volume, but a more comprehensive and qualitative approach to assessment of competency may and should be a part of the future of ERCP training.
