Prognostic Value of P-gp Expression and Related Function in Childhood Acute Leukemia

Abstract

Drug resistance is supposed to play an important role in the poor prognosis of part of the children with acute leukemia but the knowledge of drug resistance in these children is limited. Although increased expression of P-gp is found in some cases of childhood acute leukemia at initial presentation, the data are conflicting concerning the relation to clinical outcome. Most of study on the subject has been accomplished on adults and children and due to variations in methods a consensus on the results has not been established yet. Many mechanisms are playing role in drug resistance, the need to evaluate the functional aspects together with P-gp levels has been reported. The aim of this study is to evaluate the multidrug resistance in vitro and investigate the correlations with clinical findings. To evaluate the clinical importance of P-gp, its correlation with age, sex, morphology, immuno-phenotyping, hemogram, remission and outcome have been examined. We have measured the initial diagnosis, remission and relapse levels of P-gp by binding a specific monoclonal antibody (UIC2) and functional properties of the drug resistance with flow cytometry in childhood ALL and AML.
