Abstract

The expression of male sexuality encompasses significantly more than the number of times per month one has intercourse, or the ability of a man to have an erection. At the same time, however, clinicians must recognize that many middle-aged and older men place significant emphasis upon their ability to have penetrative intercourse, and that any concerns ED must be taken seriously. Population-based studies suggest that nearly two out of three men will experience ED at some point in their lives, and that one in six American men will develop prostate cancer at some point in their lives. ED can be associated with heart disease, diabetes, obesity, various prescription and OTC medications, as well as psychological disorders including depression, anxiety, and substance abuse. Additional issues associated with men’s sexuality and aging include screening and treatment for prostate enlargement and cancer, treatments for ED including the myths and misconceptions often associated with PDE-5 inhibitors including Viagra and Cialis, and body image concerns ranging from distress about musculature to male pattern baldness. Clinicians can play an essential role in providing middle-age and older male clients with the ability to seek appropriate medical consultations, allay anxiety about various diagnostic procedures, manage psychological problems associated with sexual dysfunction, address issues related to sexual identity and prowess, and involve partners in assessment and treatment. An integrated approach that incorporates psychological, medical, and educational components in both an individual and couples context appears most effective in helping cope with issues related to their sexuality and aging.
