Zusammenfassung
Histopathologische Untersuchungen erfolgen üblicherweise mit einem Lichtmikroskop. Medizinhistorisch betrachtet wurden mit genau diesem Medium erstmals infektiöse Mikroorganismen beobachtet: Die Pionierarbeiten des niederländischen Naturforschers Antony van Leeuwenhoek (1632–1723) gelten heute als Geburtsstunde der Mikrobiologie. Aber obwohl Bakterien, und andere Krankheitserreger, grundsätzlich mit einem Lichtmikroskop erkennbar sind, so stehen den Möglichkeiten einer histopathologischen Diagnostik von Infektionskrankheiten doch auch Grenzen gegenüber. Diese ergeben sich v. a. durch die tatsächliche Größe von Infektionserregern. Bei einer üblichen Dicke der Gewebeschnitte von 4–6 µm sind im histologischen Präparat vornehmlich jene Erreger erfassbar, die größer sind als 4 µm. Kleinere Mikroorganismen, speziell Bakterien, sind in histologischen Schnittpräparaten meist kaum sichtbar, wohl aber in zytologischen Präparaten von Flüssigkeiten.
Vor diesem Hintergrund erfolgt die histopathologische Diagnostik von infektiösen Magen-Darm-Erkrankungen teilweise mittels direktem Erregernachweis und andernteils indirekt anhand einschlägig charakteristischer reaktiver Veränderungen im Gewebe (⊡ Tab. 7.1). Ihre praktische Wertigkeit liegt aber nicht alleine darin, eine möglichst präzise Infektionsdiagnose zu erbringen, sondern auch darin, andere Erkrankungen abzugrenzen (von Herbay 1999).
