Zusammenfassung
Der Begriff Präkanzerosen umfasst alle Veränderungen, die mit einem erhöhten Krebsrisiko einhergehen und fakultativ oder obligat Vorstadien eines Karzinoms sind.
Während es sich bei präkanzerösen Konditionen um epidemiologisch bzw. klinisch-statistisch definierte Erkrankungen handelt, sind präkanzeröse Läsionen als histopathologisch abgrenzbare Karzinomvorstufen definiert. Intraepitheliale Neoplasien (IN; früher: Dysplasien) sind präkanzeröse Läsionen, die mikroskopisch durch Störungen der Gewebsarchitektur, der Zytologie und der Differenzierung gekennzeichnet sind (WHO 2000). Molekularbiologisch sind IN durch genomische Instabilität und Akkumulation genomischer Veränderungen charakterisiert. Das mit den IN assoziierte Krebsrisiko hängt von ihrer histologischen Graduierung (niedrig- vs. hochgradig), dem betroffenen Organ und den zugrunde liegenden Erkrankungen ab. Die Diagnostik der IN im Gastrointestinaltrakt ist von zentraler Bedeutung für die Karzinomprävention und -früherkennung.
