Zusammenfassung
Das Ovarialkarzinom tritt vornehmlich in der Postmenopause auf und weist die höchste Mortalität aller gynäkologischen Malignome auf.
Liegt bei einer Patientin klinisch der Verdacht auf einen Adnextumor vor, ist in jedem Fall eine operative Abklärung nötig. Mit Ausnahme des präoperativen Ultraschalls sind weitere Therapie verzögernde Bildgebende Untersuchungen nicht indiziert. Entscheidend für die weitere Prognose ist die maximale Reduktion des Tumors möglichst in der ersten operativen Sitzung.
Im Vordergrund steht ein primäres exaktes operatives Staging, um das korrekte FIGO-Stadium und damit das weitere Vorgehen v. a. in Form einer Chemotherapie festlegen zu können. Die postoperative Chemotherapie besteht meist aus der Kombination von Carboplatin und Paclitaxel.
Solange die Erkrankung platinsensitiv ist, können platinhaltige Therapien oft über mehrere Jahre eine gute Tumorkontrolle erreichen.
Zur Überwachung des Therapieerfolgs eignet sich in den meisten Fällen der Serum-Tumormarker CA-125.
