Zusammenfassung
Das chronische Cor pulmonale ist eine sehr häufig verkannte Erkrankung bei Patienten mit Luftnot. Oft werden die Beschwerden dieser Patienten dann als psychogen verkannt, wenn Lungenfunktion und linksventrikuläre Pumpfunktion normal sind. Durch Zunahme der Patienten mit chronisch obstruktiver Bronchopneumopathie ist zu erwarten, dass die Zahl der Patienten mit pulmonaler Hypertonie ebenfalls steigen wird.
In der Therapie der primär pulmonalen Hypertonie hat sich mit Einführung der inhalativen Prostazyklintherapie sowie mit oral wirksamen Endothelinantagonisten ein völlig neues Therapieprinzip etablieren können (Hoeper et al. 2000). Neben der symptomatischen Besserung lassen diese Behandlungsoptionen hoffen, das auch die sehr schlechte Prognose der betroffenen Patienten gebessert werden kann.
