Zusammenfassung

Seit Penicillinderivate zur Therapie und Prophylaxe von Streptokokkeninfekten breit angewandt werden, sind die sog. rheumatischen Herzklappenfehler wesentlich seltener geworden. Degenerative Herzklappenfehler — insbesondere kalzifizierende Aortenstenosen und Mitralinsuffizienz bei Funktionsstörung des subvalvulären Klappenapparates — werden dagegen parallel dem Zugewinn an allgemeiner Lebenserwartung häufiger beobachtet (Horstkotte u. Loogen 1987). Nosologisch bedeutsam sind daneben infektiöse und ischämische Herzklappenfehler, Stenosen auf dem Boden angeborener bikuspider, primär nicht stenotischer Aortenklappen und myxomatöse Degenerationen der Mitralklappe. Als Ausdruck dieses ätiologischen Wandels hat das mittlere Alter der symptomatischen Patienten in den letzten 30 Jahren deutlich zugenommen.

Die Zahl der erstmals diagnostizierten wie der pro Jahr operierten Herzklappenfehler (1982: n = 3788; 1992: n = 9284; 2000: n = 15.590) ist in Deutschland stetig steigend, eine Abnahme der Prävalenz in absehbarer Zukunft nicht zu erwarten (Kalmar u. Irrgang 2001).
