Luxation métacarpophalangienne postérieure du pouce : la manœuvre de Farabeuf

Résumé

La luxation postérieure de l’articulation métacarpophalangienne du pouce fait suite à un traumatisme en hyperextension. Le risque est l’incarcération de la plaque palmaire, incluant les os sésamoïdes, en arrière du métacarpe. Il s’agit alors d’une luxation complexe, dont la réduction peut être impossible et nécessiter une prise en charge chirurgicale. La réduction d’une luxation simple, sans incarcération de la plaque palmaire, doit se faire exclusivement par la manoeuvre de Farabeuf, afin d’éviter sa transformation en luxation complexe.

Abstract

The dorsal dislocation of the metacarpophalangeal joint of the thumb is caused by a hyperextension injury. The dislocation is complex if the volar plate and sesamoid bones are entrapped within the metacarpophalangeal joint space. This latter is difficult to reduce and often require surgical intervention. A simple dislocation should be reduced only by Farabeuf maneuver to avoid turning it into complex dislocation.
