De helft van de zelfstandig wonende kwetsbare ouderen ontvangt geen thuiszorg (huishoudelijke hulp, persoonlijke verzorging of verpleegkundige hulp) uit de Wmo of AWBZ. Onderzocht is welke kwetsbare ouderen geen Wmo/AWBZgefinancierde thuiszorg ontvangen en wat de determinanten van niet-gebruik zijn. De gebruikte gegevens zijn afkomstig van 310 zelfstandig wonende kwetsbare personen van 65 jaar en ouder. Kwetsbaarheid werd vastgesteld met de Tilburg Frailty Indicator (TFI). De analyses tonen dat niet-gebruikers gemiddeld jonger zijn en vaker samenwonen met huisgenoten dan kwetsbare ouderen die thuiszorg ontvangen. Ook de aanwezigheid van informele hulp vergroot de kans dat geen professionele thuiszorg geboden wordt. De kans op thuiszorg is hoger bij de aanwezigheid van ernstige lichamelijke beperkingen, bij ouderen met een voorkeur voor hulp door een zorginstelling en bij een ziekenhuisopname. Het ontbreken van Wmo/AWBZ-gefinancierde thuiszorg hoeft niet altijd een probleem te zijn voor zelfstandige kwetsbare ouderen. Niet alle kwetsbare ouderen hebben zulke ernstige beperkingen dat verzorging nodig is. Bovendien regelt een deel van de ouderen zelf informele of particuliere hulp. De kwetsbare ouderen zonder thuiszorg in deze studie hadden bijna allemaal recent contact met een hulpverlener uit het (para-)medische circuit. Deze hulpverleners (met name de huisarts) kunnen een rol spelen bij de monitoring van zelfstandig wonende kwetsbare ouderen.
Abstract
Frail elder persons without home care
In the Netherlands half of the community-dwelling frail elder persons do not receive home care, i.e. collectivelyfinanced domestic help, personal care or nursing. This study identifies the frail elder persons without collectivelyfinanced home care and the determinants of non-use. Cross-sectional survey data (2007) are used from 310 community-dwelling frail persons of 65 years and older. Frailty was assessed by the Tilburg Frailty Indicator (TFI). The analyses show that non-users are on average younger and more often live with a housemate as compared to frail elder persons who do receive home care. The presence of informal help also increases the risk of non-use of professional home care. The risk of home care is increased in the presence of serious physical disabilities, among persons with a preference for help by a professional care agency and in the case of hospitalization. The absence of collectivelyfinanced home care is not by definition a problem for community-dwelling frail elder persons. Not all frail elder persons have such serious physical and functional disabilities that care is required. In addition, some elder persons arrange care by themselves (informal or private). Almost all of the frail elder persons without home care in this study had recent contact with a (para-)medical care worker. These care workers – the general practitioner in particular – could play a role in the monitoring of communitydwelling frail elder persons.
