The authors have noticed an error and omission since publication of their manuscript.
Under the methods section (sixth paragraph, second sentence) the text “Each symptom/sign was assessed on scale of 0–3 (0 = absent, 1 = mild, 2 = moderate, 3 = severe) with a maximum score of 18.” should read: “Each symptom/sign was assessed on scale of 1–4 (1 = absent, 2 = mild, 3 = moderate, 4 = severe) with a maximum score of 18.”
