Résumé
Le syndrome de Budd-Chiari compliquant une sarcoïdose hépatique est rare. De nos jours, il n’existe pas d’arguments consensuels pouvant expliquer le mécanisme d’association. De 1978 en 2009, cinq cas ont été publiés. Nous rapportons ici une nouvelle observation.
Patient de 33 ans, admis pour cholestase ictérique, les examens d’imagerie retrouvaient une rate d’aspect normal et mettaient en évidence une hypertrophie du lobe gauche et du segment I du foie, une thrombose des veines sushépatiques avec un important réseau veineux collatéral et de multiples adénopathies profondes abdominales et médiatisnales dont l’examen anatomopathologique montrait des images caractéristiques de sarcoïdose. Le traitement par corticothérapie associé aux anti vitamine K a permis une régression des signes. En conclusion: bien que rare, la sarcoïdose peut être considérée comme une des étiologies du syndrome de Budd-Chiari comme l’illustre notre observation.
Abstract
Budd-Chiari syndrome is a rare complication of hepatic sarcoidosis. Today, there is no consensual argument that could explain the mechanism of this association. From 1978 to 2009, five cases have been published. Here we report a new observation
The patient of 33 years admitted for icteric cholestasis whose imaging tests showed a normal aspect’ rate with marked hypertrophy of the left lobe and of segment I of the liver, thrombosis supra-hepatic vein with a highly developed collateral network, and multiple and profound abdominal and mediastinal adenopathies; the anatomopathological examination showed characteristic images of sarcoidosis. The treatment with corticosteroids associated with antis vitamin K allowed a regression of the symptoms. To conclude we can affirm that although rare, sarcoidosis can be considered as one of the etiologies of Budd-Chiari syndrome as illustrated by our observation.
