Zusammenfassung
Die kathetergestützte Ablation insbesondere von Vorhofflimmern hat sich mittlerweile als Therapieoption etabliert und wird auch im Rahmen der internationalen Leitlinien als Standardverfahren akzeptiert.
Wesentliche Limitation der manuellen und meist röntgenologisch geführten Katheterablation ist neben der Strahlenexposition für Patient und Untersucher die mangelnde dreidimensionale Darstellung der teils komplexen Anatomie der Vorhöfe oder Kammern. Außerdem ist die Persistenz der erreichten Läsionen und somit z. B. der Isolation der Pulmonalvenen mittels konventioneller Katheterführung bislang noch einer der wesentlichen Gründe, die bessere Ergebnisse nach der Behandlung von Rhythmusstörungen mittels Katheterablation verhindern.
In den letzten Jahren haben wir nicht nur viel Wissen um die elektrophysiologischen Mechanismen vieler Rhythmusstörungen hinzugewonnen, es haben sich außerdem einige technische Neuerungen ergeben, die womöglich helfen können, akute und dauerhafte Ablationsergebnisse zu verbessern.
Der folgende Artikel stellt die heutigen Möglichkeiten des dreidimensionalen Mappings einschließlich der Integration von CT und MRT dar und gibt eine Übersicht über die verfügbaren Methoden zur robotisch bzw. magnetisch geführten Katheterablation.
Abstract
Catheter ablation of atrial fibrillation has evolved as a widely accepted therapy approach and is now also incorporated in the current guidelines.
A major limitation consists of the limited three-dimensional visualization of the complex three-dimensional structures in the left atrium since most procedures have routinely been performed using fluoroscopy alone. Another unsolved problem is the limited durability of lesions sets performed with radiofrequency ablation and therefore somewhat disappointing long-term ablation results besides fluoroscopy exposition for patient and operator as required for safe catheter manipulation.
In the recent years we have gained substantial insight with respect to arrhythmia mechanism. At the same time new techniques and developments have become available to improve catheter ablation results.
The present article summarizes the available opportunities with respect to three-dimensional mapping including CT/MRI image integration and gives an overview of the robotic and magnetic systems available for catheter ablation.
