Résumé

Le traitement de l’algie vasculaire de la face (AVF) associe dans un premier temps un traitement de crise: sumatriptan injectable et/ou oxygène, et un traitement de fond: vérapamil et/ou sels de lithium. Avant de passer la main au chirurgien, en cas de crises très nombreuses et/ou épuisement ou non-fonctionnement du traitement de fond, on pourra avoir recours à un traitement transitionnel: corticothérapie per os ou en infiltration sous-occipitale. En cas d’AVF réfractaire, le traitement chirurgical consistera en un traitement lésionnel ou une stimulation cérébrale profonde, des nerfs grands occipitaux ou du ganglion sphénopalatin.

Abstract

The treatment of cluster headaches initially involves treatment of an attack: sumatriptan injection and/or oxygen, and prophylactic treatment: verapamil and/or lithium salts. Before turning to surgery, in cases of very frequent attacks and/or the running out or failure of prophylactic treatments, transitional treatments can be an option: oral or suboccipital infiltration of corticosteroids. In cases of refractory cluster headaches, surgical treatment can be lesional, or deep brain stimulation, occipital nerve stimulation or sphenopalatine ganglion stimulation.
