Zusammenfassung
Die Nierentransplantation ist ein effektives Verfahren, um das Überleben von Patienten mit terminaler Niereninsuffizienz zu verlängern und die Lebensqualität zu erhöhen. Die Entwicklung neuer Immunsuppressiva in den vergangenen Jahrzehnten eröffnete neue Therapiemöglichkeiten und führte zur signifikanten Reduktion der akuten Abstoßungsereignisse. Die Transplantationslangzeitergebnisse haben sich jedoch nicht im selben Maße weiterentwickelt. Biologika werden heute routinemäßig bei der Nierentransplantation verwendet. Die auf Biologika (Rezeptorantagonisten oder lymphozytendepletierende Antikörper) basierende Induktionstherapie verbessert das Transplantatüberleben und senkt das Risiko für akute Abstoßungen. Kostimulationsblocker, die gezielt die T-Zell-Aktivierung blockieren, könnten zukünftig eine Calcineurininhibitor (CNI)-freie Langzeitimmunsuppression erlauben; die bisherigen Daten aus randomisierten, multizentrischen Studien sind ermutigend. Biologika sind bei der Therapie akuter Abstoßungen von hoher Bedeutung. Akute, steroidresistente zelluläre Abstoßungen erfordern den Einsatz lymphozytendepletierender Biologika; bei humoralen Abstoßungen kann die B-Zell-Depletion eine Therapieoption sein. Komplementblocker können bei therapierefraktären Verläufen erwogen werden. Insgesamt stellen die verfügbaren Biologika eine sinnvolle Ergänzung zur heutigen immunsuppressiven Standardtherapie mit CNI, Mycophenolat-Mofetil und Steroiden dar.
Abstract
Renal transplantation increases survival and quality of life of patients with end-stage renal failure. The development of new immunosuppressive agents has led to new therapeutic strategies and to significant reduction of episodes with acute allograft rejection. However, the long-term outcome of patients after renal transplantation did not improve to the same extent. Biologics are nowadays used routinely in renal transplantation. Induction therapy which is based on biologics improves survival and decreases the risk of acute rejection. Blockade of costimulation inhibiting T-cell activation could allow calcineurin inhibitor (CNI)-free long-term immunosuppression as has been recently demonstrated in the randomized multicenter BENEFIT trial. Biologics are important therapeutic agents to treat acute rejection. Steroid-resistant cellular rejection should be treated with lymphocyte-depleting biologics. In cases of humoral rejection, B-cell depleting agents may be considered. Complement blockade can be an alternative therapeutic option in patients with a refractory course of disease. In summary, biologics are potent and effective agents to complement the standard immunosuppressive therapy with CNI, mycophenolate mofetil and steroids.
